As a user of this system, I understand that I will be accessing private health care information that must be used only for legal and appropriate business purposes and must be kept confidential. Furthermore, access to the information will be through equipment and software (such as a personal computer and a browser) which may cache or store information locally. I accept full responsibility for securing and maintaining the privacy of any such information once it is sent to my machine. This includes, but is not limited to information on display monitors, memory, storage devices including backups, and printed information. I agree to protect from unauthorized use and keep private any passwords or other authentication credentials which are used to identify me and limit my access to that which is required to perform legal and authorized job responsibilities. I will immediately report any known or suspected disclosures of passwords or other authentication credentials to my local security coordinator and take steps to have them changed. I will notify my security coordinator if I no longer need access to the system or if I should obtain unauthorized access under any circumstances, including administrative error, job reassignment, etc. I agree not to misuse my access and accept full responsibility for all activity that occurs on my account. I understand that I should never share or provide my password with others. This includes helpdesk and security support personnel. No BCBSNC employee or agent will ever ask me for my password and if they do, I am under no obligation to supply it.
